Personal Information Removal Request Form

For privacy reasons, you may have the right to ask for certain personal information relating to you to be removed.

This form is for requesting the removal of specific results for queries that include your name from Google Search. Google LLC is the controller responsible for the processing of personal data carried out in the context of determining the results shown by Google Search, as well as handling delisting requests sent through this form.

If you want to request removal of personal information from another Google product, please submit a request through that product's form, which you can reach at our Removing Content From Google page. For example, if you want to request removal of personal information from Blogger, please submit a request on the relevant Blogger form.

When you make your request, we will balance the privacy rights of the individual concerned with the interest of the general public in having access to the information, as well as the right of others to distribute the information. For example, we may decline to remove certain information about financial scams, professional malpractice, criminal convictions, or public conduct of government officials.

* Required field

Your own name, even if you are making the request on behalf of someone else who you are authorized to represent. If you are representing someone else, you must have the legal authority to act on their behalf.

If you are submitting this request on behalf of someone else, please specify your relationship to that person (for example: "parent", "attorney"). We may ask for documentation confirming that you are authorized to represent this person.

If you (or the relevant individual) have already asked us to remove URLs containing similar content, we can help you more quickly if you reply to the email we sent you (or the relevant individual) instead of sending a new notice.

If you would rather send us a new notice, enter the 14-digit reference number from the previous request. As an example, the format will be similar to 1-1111000001111. You can find it in the subject line of the email we sent in response to the previous request.

If this notice concerns multiple reasons for infringement, please submit only the first one below. Then, click the "Add a new group" link below the text boxes to add another reason.

This should be the name that, when used as a search query, produces the results you would like to delist. If you wish to submit multiple names (e.g. if your maiden name differs from your current last name), put a "/" between the names. For example, "John Smith / John Doe".

Please read the following statements, and check the boxes to confirm that you have read and acknowledge them.

Google LLC will use the personal information that you supply on this form (including your email address and any ID information) and any personal information you may submit in further correspondence for the purposes of processing your request and meeting our legal obligations. We may share details of your request with data protection authorities, but only when they require these details to investigate or review a decision that we have made. That will normally be because you have chosen to contact your national DPA about our decision. Where URL(s) have been removed from our Search results as a result of your request, we may provide details to the relevant webmaster(s) of the URL(s) that have been removed.

Please note that if you are signed into your Google Account, we may associate your submission with that account.
